Arizona Sundry Notice (5-AZ-b)

In Arizona, a geothermal developer utilizing an injection well must submit a Sundry Notice form to the Arizona Oil and Gas Commission. This form shall be filed weekly on all drilling, completion, recompletion and workover operations. Ariz. Admin. Code § R12-7-179.
Arizona defines geothermal wells as Class V wells that require injection well permits. Ariz. Admin. Code § R12-7-175.
This notice must be submitted on the Arizona Oil and Gas Conservation Commission – Sundry Notice form.
Sundry Notice Process
5-AZ-b.1- File Sundry Notice
The developer must file a Sundry Notice weekly using the Arizona Oil and Gas Conservation Commission – Sundry Notice form.
