SURF Personnel: George Vandine
Location(s): 4850L W. Drift
Summary:
· Pressure-tested OT and OB well caps per JHA EGS-020
· Added quick-setup epoxy mixture layer to OT well cap
Lead Researcher: Mathew Ingraham (mdingr@sandia.gov; 505-595-5230)
Documents or Procedures: JHA EGS-009, JHA EGS-017 (ver. 2), JHA EGS-020
Inspections: OB well cap passed pressure test. Leak was found in OT well cap, needed additional layer of epoxy.
Materials Receiving/Shipping: Epoxy chemicals were reported to arrive but did not make it to the 4850. Nitrogen was also not delivered.
Comments: When OT well cap was found not to hold pressure, cap was investigated at lower pressures to determine leakage pathway. Pathway was found to be cable feedthroughs/flange seal of this low-angle well, became clear that only issue was that additional epoxy was needed. After discussion, a suitable quick-setup (overnight) mixture of epoxy was identified. An ~1-inch layer of this epoxy layer was added to seal the leakage pathway and allowed to cure overnight.
Recommendations: N/A
Irregularities: Scheduled nitrogen bottle shipment was delayed, worked with Markus H & Jaret H to make sure tomorrow’s planned epoxy injection would not be compromised.
Acts of Safety: Pressure test protocol was key in safely and effectively identifying OT well cap leakage pathway. Performing this work ahead of pressurized epoxy injection allowed for suitable solution to be found and avoided situation of epoxy injection into improperly sealed well cap.
Near Misses or Incidents: N/A
